Job summary

Centralus Health in Elmira, NY is seeking an RN Diabetes Nurse Educator to deliver professional nursing, consultation, and educational services for persons with diabetes in an outpatient setting.

You will develop curriculum on health, assess patient needs, and work with the care team to coordinate disease management across ages. This role reports to Outpatient Services-Director of Nursing and Clinical Services, and requires NY RN license; CDE preferred.

Qualifications

• Graduate of an approved RN program with NY license.
• Certified Diabetes Educator (CDE) or the ability to obtain certificate within two years of employment.
• Excellent communication and writing ability.
• Basic Life Support (BLS) required within 90 days of hire.
• Previous nursing experience in a medical office setting preferred.

Responsibilities

• Provide comprehensive diabetes education to patients, families, and staff.
• Assess, monitor, and implement individualized treatment plans for patients with diabetes.
• Collaborate with physicians, care coordinators, and interdisciplinary team members to ensure coordinated, high-quality patient care.
• Perform patient assessments, triage, and care planning while identifying changes in patient condition.
• Deliver patient-centered education related to medications, procedures, chronic disease management, preventive care, and community-based support programs.
