Qualifications

• Preferred 2–4 years in case management or acute care nursing.

• Current RN licensure in applicable state(s).

Responsibilities

• Organize and provide care across assessment, planning, intervention, and evaluation.
• Support physician and interdisciplinary team to advance patient progress toward discharge.
• Coordinate utilization management, care facilitation, and discharge planning.
