Job summary

Novant Health is seeking an RN Case Manager to lead complex discharge planning and coordinate care with physicians and the healthcare team. The role focuses on proactive, safe discharge plans and collaboration with MSW for psychosocially complex cases.

You will act as a resource for discharge planning and care coordination for medically complex cases, maintaining knowledge of community resources and post-acute care options. Weekend/holiday coverage may be required.

Qualifications

• RN licensure in state of employment is required.
• Education: 2 Year / Associate Degree; 4 Year / Bachelor’s preferred.
• Experience: two or more years in healthcare; one year case management preferred.
• Weekend and holiday coverage may be required.

Responsibilities

• Lead the discharge planning process and coordinate care with the healthcare team.
• Assess, plan, implement and evaluate proactive/complex/safe discharge plans.
• Collaborate with MSW for psychosocially complex cases and resource guidance.
• Maintain knowledge of community resources and post-acute care options.
