Job summary

WelbeHealth PACE in North Hollywood, CA is hiring a Case Manager, RN to guide participant care within an interdisciplinary team. You will conduct clinic nursing assessments, develop and follow personalized care plans, and coordinate transitions of care.

Requirements include BSN preferred, CA RN license, BLS/First Aid, and at least two years of nursing experience with frail or elderly populations. The role offers a base salary plus bonus, a comprehensive benefits package, and a Monday–Friday

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Unencumbered California RN license.
• Current BLS/First Aid certification.
• Two years of nursing experience with frail/elderly population preferred.
• CPR and First Aid certification within 180 days of hire (preferred at hire).

Responsibilities

• Conduct nursing assessments to determine participant needs and goals.
• Deliver and document nursing interventions per care plans.
• Collaborate with IDT to coordinate participant care.
• Educate participants and families on disease management and self-care.
