WelbeHealth PACE seeks a Case Manager, RN to guide participant care as part of the interdisciplinary team. You will conduct nursing assessments, develop patient care plans, and support transitions of care coordination.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Unencumbered California RN License.
• Current Basic Life Support (BLS)/First Aid Certification required.
• Two years of nursing experience with frail or elderly population preferred.

Responsibilities

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals and participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions as per care plans, including maintaining a safe environment and administering medications.
• Collaborate with the WelbeHealth PACE Physician and IDT to coordinate care needs and communicate with participants and families to revise care plans.
• Instruct participants and families in disease process, self-care techniques, and prevention strategies.

Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.**This role is different because Case Manager, RNs at WelbeHealth:*** Work a consistent Monday through Friday schedule – no weekends and no on call* Build relationships with participants rather than providing short-term care* Career growth opportunities**We care about our team members.
