Job summary

Fairview Health Services is seeking a Nurse Case Manager for the Vascular Clinic. This role requires travel to vascular clinics in Maplewood, Woodbury, Wyoming, and surgery centers in Minneapolis and Maple Grove, with 7:30 a.m. – 5:00 p.m. availability. The position offers a casual schedule in a collaborative clinic environment.

You will triage nursing calls, educate patients, and support wound/vascular care plans while collaborating with physicians, LPNs, CMAs, and patients to optimize outcomes.

Qualifications

• Associates Degree in Nursing required.
• Basic Life Support (American Heart Assoc or Red Cross) required.
• Registered Nurse license in Minnesota required.
• Wound Care/Acute Care experience or Vascular Certification preferred.

Responsibilities

• Develop and implement care plans under physician/nurse practitioner guidance.
• Triage nursing calls and coordinate patient education and needs.
• Document wound and vascular assessments for new and follow-up visits.
• Coordinate with care team and external providers to ensure plan adherence.
• Maintain compliance with applicable laws, regulations and policies.

Job Overview

Fairview has an immediate opening for an RN Case Manager to join our team at the Vascular Clinic. Join Fairview, the largest healthcare system in the state! The Vascular Clinic is a growing and collaborative clinic. This position will travel to our vascular clinics in Maplewood, Woodbury, and Wyoming, as well as our surgery centers in Minneapolis and Maple Grove, and will need to be available between 7:30 a.m. and 5:00 p.m. This position is a casual role.

The Vascular Center Case Manager RN works in a variety of different roles and clinics in the department. Case management of Vascular, Vein and Wound patients ensuring all the needs and educate of the patient are needed. Performs triaging on incoming nursing calls and questions Independent Wound and Education Visits with patient Team collaboration between Supervising Physician, LPN's, CMA's and Patients.

Responsibilities

• Under the direction of the Vascular Center physician/nurse practitioner, prepares and implements a care plan according to the Vascular Center written orders and updates appropriately.
• Provides ancillary patient care needs to include, but not limited to review of laboratory and culture data; appropriate interaction with physicians and pharmacies; triaging of telephone calls; interaction with caregivers.
• Completes and documents wound and vascular assessment on both new and follow-up patients for each patient visit.
• Assess patient's educational needs.
• Provides communication between the Center and ancillary departments and care givers to facilitate compliance with vascular and wound plans of care including orthotics, physical medicine, etc.
• Implements wound and vascular patient care protocols where positive viral screens occur. Coordinates the ordering of clinical supplies and PDWHF as needed.
• Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
• Actively participates in creating and implementing improvements.
• Performs other responsibilities as needed/assigned.
