Job summary

Fairview Health Services in Minnesota is seeking an RN Case Manager for the Vascular Clinic. This casual role travels to vascular clinics in Maplewood, Woodbury, and Wyoming, and to surgery centers in Minneapolis and Maple Grove, with work hours from 7:30 a.m. to 5:00 p.m.

You will develop and implement care plans, triage nursing calls, perform wound and vascular assessments, and educate patients while collaborating with physicians, LPNs, CMAs, and caregivers.

Qualifications

• Associates Degree in Nursing
• Basic Life Support (American Heart Assoc or Red Cross)
• Registered Nurse MN

Responsibilities

• Prepare and implement a care plan under physician oversight and update accordingly.
• Provide wound and vascular assessments and document findings.
• Triaging telephone calls and coordinating care with physicians and pharmacies.
• Educate patients and assess educational needs.
• Coordinate care with orthotics and rehabilitation services as needed.
• Maintain compliance with relevant laws and internal policies.
