Job summary

Cleveland Clinic Weston Hospital in Florida is seeking an experienced RN Case Manager to coordinate transitions of care and advocate for patients. You will collaborate with interprofessional teams across the continuum of care to facilitate effective transitional care coordination, develop individualized care plans, and empower patients and families to successfully self-manage their care.

The role supports hospital throughput, coordinating complex discharges, and providing post-discharge care

Qualifications

• Graduate from an accredited school of Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification via AHA or Red Cross.
• Three years of recent RN clinical experience.

Responsibilities

• Coordinate transitional care across the continuum of care.
• Advocate for patients and families and conduct holistic assessments.
• Develop individualized care plans and empower self-management.
• Lead in patient rounding, huddles, and care coordination meetings.
• Complete effective handoffs to the next level of care or community.
• Support hospital throughput and discharge planning to ensure safe transitions.
