Job summary

This full-time position requires RN licensure, BLS certification, three years of RN experience and at least one year of pediatric RN experience.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state RN licensure.
• BLS certification through AHA or Red Cross.
• Three years of recent RN experience.
• At least one year caring for pediatric patients as an RN.

Responsibilities

• Collaborate with inter-professional teams to coordinate transitional care across the continuum.
• Facilitate effective handoffs to next level of care or community to ensure continuity of care.
• Participate and lead patient rounding, huddles and committee meetings.
• Identify and mitigate barriers to patients' transitions of care.
• Develop comprehensive, individualized transitional care plans with patients and families.
