Job summary

Elevance Health seeks a Nurse Case Manager to perform telephonic and field-based care management across South Central and South West Kansas. You will assess, coordinate, and implement care plans for members with complex needs, linking hospitals, clinics, and home/services to optimize outcomes.

Responsibilities include telephonic and on-site assessments, developing care plans, coordinating authorizations/referrals, and monitoring effectiveness with Medical Directors. Travel 10–15% is required.

Qualifications

• Requires BA/BS in a health-related field and minimum of 3 years of clinical experience.
• Current, unrestricted RN license in the state of Kansas is required.
• Computer proficiency is preferred.

Responsibilities

• Perform care management assessments and coordinate care telephonically and on-site.
• Develop, implement, and monitor care plans to optimize member health.
• Coordinate authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Collaborate with Medical Directors and Physician Advisors on care management plans.
• Travel 10–15% to visit members in person across South Central to South West Kansas.
