Job summary

Elevance Health is seeking an RN Case Manager for telephonic and field-based work across South Central and South West Kansas. The role requires travel to member sites, with occasional office attendance, and residency in Kansas.

You will assess, plan, coordinate, and monitor care across the continuum, using telephonic and on-site visits to optimize outcomes. A BA/BS in a health-related field and a Kansas RN license with 3 years of clinical experience are required.

Qualifications

• Requires BA/BS in a health-related field with minimum 3 years of clinical experience.
• Current, unrestricted RN license in Kansas is required.
• Ability to travel 10–15% and work in field settings.

Responsibilities

• Assess, develop, implement, coordinate and monitor care plans for members.
• Perform telephonic and on-site care coordination and discharge planning.
• Coordinate care with internal and external providers within benefits structure.
• Monitor outcomes and adjust care plans; interact with Medical Directors as needed.
• Facilitate authorizations/referrals and address provider/service issues.
