Elevance Health is hiring an RN Case Manager for a telephonic/field role based in Overland Park, Kansas. The position supports members with complex and chronic needs through assessments and care management plans, coordinating resources and authorizations to improve outcomes.

The role requires a Kansas RN license, a BA/BS in a health-related field, and at least 3 years of clinical experience. Travel to South Central and South West Kansas is expected 10-15% of the time.

Qualifications

• Education: BA/BS in a health-related field required
• Experience: Minimum of 3 years of clinical experience required
• Licenses & Certifications: Current, unrestricted RN license in the state of Kansas required

Responsibilities

• Perform care management within the scope of licensure for members with complex and chronic care needs.
• Conduct assessments virtually and in person to identify individual needs.
• Develop and implement care management plans to address objectives and goals.
• Facilitate authorizations/referrals as appropriate within benefits structure.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans.
• Interface with Medical Directors and Physician Advisors on treatment plans.
• Negotiate rates of reimbursement as applicable.
• Assist in problem solving with providers, claims, or service issues.
• Travel 10-15% within South Central to South West Kansas to visit members.
