Qualifications

• Must be a graduate from an accredited school of Nursing.

• Minimum two years of recent RN experience in inpatient case management and ambulatory RN CM experience.
• Proven background in managing high-risk patients post-discharge; telephonic outreach with insured populations.
• Proficiency in information systems including Word and Excel.
• Excellent interpersonal and communication skills.
• Team-oriented and collaborative work style.

Responsibilities

• Perform telephonic outreach to high‑risk patients to help prevent avoidable events.
• Coordinate post-discharge care with payers, home health, DME, and skilled nursing facilities.
• Support case management efforts for inpatient and ambulatory patients.
