Job summary

UC San Diego Health is seeking a registered nurse with case management experience to perform telephonic outreach to high‑risk patients and coordinate post‑discharge care. Strong EPIC and care port knowledge are highly valued, along with excellent communication and teamwork.

The role supports the Population Health Care Connections Hub and requires California RN licensure, a BSN or higher, and a flexible schedule including on-site one day per week and remote work as needed.

Qualifications

• Graduate from an accredited school of Nursing.
• RN license issued by the State of California.
• Minimum two (2) years of recent RN inpatient case management and ambulatory RN CM experience.
• Experience with telephonic outreach to high‑risk patients and coordination with insurance providers.

Responsibilities

• Perform telephonic outreach to high‑risk patients discharged from inpatient care.
• Assist patients with care coordination and resource linkage (DME, HH, home health, SNF).
• Collaborate with care teams using EPIC and Care Port to optimize outcomes.
