Job summary

UC San Diego Health is seeking a registered nurse with case management experience to perform telephonic outreach for high-risk patients and coordinate care post-discharge.

Ideal candidates have IN inpatient case management and ambulatory CM experience, with strong communication skills and proficiency in EPIC and Care Port. A hybrid schedule with on-site one day per week is supported, and a CA RN license is required.

Qualifications

• Graduate from an accredited school of Nursing.
• RN license issued by the State of California.
• Minimum two years of inpatient case management and ambulatory CM.
• Experience with post-discharge high-risk patients preferred.
• Proficiency with Word and Excel.
• Strong interpersonal and communication skills.
• Ability to work in a team environment.

Responsibilities

• Perform telephonic outreach to high-risk patients to help prevent avoidable events.
• Coordinate care with providers, payers, and community resources.
• Utilize EPIC and Care Port to support care coordination.
• Work a hybrid schedule with on-site presence as required.
