Bellway Health and Rehabilitation Center in Selma, AL is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate resident care from admission through discharge, and to support positive clinical outcomes within our skilled nursing team.

The ideal candidate holds an Alabama RN license, has experience in long-term care, strong assessment and communication skills, and can collaborate with an interdisciplinary team while ensuring compliance with regulations.

Qualifications

• Active RN license in Alabama.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Strong assessment, organization, and communication skills.
• Ability to collaborate with an interdisciplinary team.
• Knowledge of care planning, discharge planning, and regulatory requirements.
• Current CPR certification or ability to obtain as required.

Responsibilities

• Coordinate and monitor residents' plans of care from admission through discharge.
• Collaborate with nursing, therapy, social services, physicians, and other departments.
• Communicate regularly with residents and families regarding care needs and progress.
• Participate in interdisciplinary care plan meetings.
• Assist with discharge planning and transitions of care.
• Monitor changes in residents' conditions and ensure appropriate follow-up.
• Maintain accurate and timely clinical documentation.
• Help ensure compliance with facility policies and state and federal regulations.
• Advocate for residents and promote quality, resident-centered care.
