Job summary

Good Shepherd Lutheran Home in Sauk Rapids, MN seeks a Registered Nurse Case Manager to oversee a nursing home household with a strong focus on person-centered care. You will guide staff, ensure safe med administration, and coordinate admissions, discharges, and transfers.

The role requires an MN RN license, BLS/CPR/AED, and at least two years of nursing experience (LTAC preferred). Excellent English communication and leadership are essential for success.

Qualifications

• Current Minnesota RN license
• BLS certification including CPR and AED
• Minimum of two years nursing experience; long-term care experience preferred
• Excellent verbal and written communication in English; independent and team player
• Demonstrated leadership to guide staff under pressure

Responsibilities

• Oversee and manage an assigned nursing home household with person-centered care
• Guide and support household staff in meeting residents' physical, emotional, and psychosocial needs
• Monitor adherence to Standards of Nursing Practice and facility policies for med administration
• Administer medications and treatments per physician orders and policies
• Coordinate resident admissions, discharges, and transfers
• Assess residents admitted or returning from hospital; ongoing assessments for all MDS sections
• Complete the Resident Assessment Instrument (RAI) including MDS and care plans
• Initiate care plans with measurable goals reflecting resident status
• Monitor planning, scheduling, and revisions of the RAI process
• Perform RN nursing skills, including IV therapy and blood draws
• Complete case management assessment for new residents per regulatory requirements
• Process, transcribe, and implement physician orders per policies
• Maintain knowledge of emergency protocols and respond to emergencies
