Job summary

Mission Hospital in Asheville, NC is seeking a Case Manager to support patients and families through care transitions. You will coordinate services, assess needs, and partner with physicians and the care team to ensure safe, quality transitions.

The role requires 2+ years in case management or 3+ years in a clinical setting, RN licensure, and BLS. A sign-on bonus is available for qualified candidates and opportunities for mentorship and education are offered.

Responsibilities

• Provide case management for inpatient and observation patients
• Identify patients at risk during transitions of care
• Assess psychosocial, medical, and discharge needs
• Coordinate care plan and discharge planning with care team
• Collaborate with Social Services for post-acute needs
• Evaluate progression of care using InterQual criteria
• Maintain effective communications with physicians, patients, families, and partners

Job Summary And Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

• You will provide case management services for both inpatient and observation patients as assigned.
• You will Identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another
• You will perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family
• You will coordinate the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition
• You will partner with Social Services to ensure the post-acute medical needs and level of care are appropriate
• You will evaluate progression of care using evidence-based tools and approved criteria (InterQual) throughout the episode of care; escalates progression and transition of care issues through the established chain of command
• You will act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff, and outside agencies

Qualifications

• Required Experience: 2+ years experience in Case Management OR 3+ years experience in a clinical setting
• Preferred Experience: InterQual experience
• Required Education: Associates Degree in Nursing
• Preferred Education: Masters Degree in Nursing
• Required License: Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
• Required Certification: BLS
