CarePartners Health Services, a member of Mission Health, is seeking a Registered Nurse Case Manager for Inpatient Rehab in Asheville, NC. You will coordinate care, educate patients and families, and develop individualized plans that reflect each patient’s needs and goals.

You’ll work with physicians, nurses, and leaders to ensure safe transitions and high-quality, patient-centered care. The role emphasizes collaboration with the Rehab Program Director and interdisciplinary team, while upholding

Qualifications

• RN or Bachelor's Degree; work experience in lieu of degree must be obtained within 30 days of employment start date.
• RN license must be obtained within 30 days of employment start date.
• 5 years experience required.

Responsabilités

• Guide patients and families through program orientation, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to plan participation in treatment.
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations; communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare values through every patient and family interaction.
