Community Care is hiring a RN Case Manager for our Family Care Program in Waukesha, WI. This full-time role involves coordinating care for elderly and disabled adults in the community, performing home visits, and developing comprehensive care plans.

Qualifications

• Associates degree in Nursing required; Bachelor's preferred.
• Wisconsin RN license required.
• Eligibility for long-term care functional screener certification required.
• Wisconsin driver's license required.
• Minimum 2 years of nursing experience with older adults or persons with disabilities; community nursing preferred.

Responsibilities

• Completes discipline-specific initial and ongoing assessments, conducts home visits and consults with families and supports.
• Develops and maintains care plans, tracks progress, coordinates services and ensures quality and effectiveness of care.
