Job summary

Community Care is seeking a Case Manager to join our mission-driven team in Dane County, WI. This full-time role features a Monday–Friday schedule (8:00 AM–4:30 PM) and a generous $8,000 sign-on bonus to help support dedicated professionals who coordinate care for older adults and adults with disabilities in the community.

The Case Manager will collaborate with an RN Case Manager, complete assessments, develop care plans, monitor services, and ensure accurate documentation while traveling

Qualifications

Bachelor's Degree in a Human Services or closely related area required.
Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
Eligibility for long-term care functional screener certification is required.
Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Responsibilities

Manages an assigned caseload of program members in collaboration with an RN Case Manager
Completes comprehensive member assessments, member centered care plans, & functional screens
Provides ongoing care coordination and quality assurance monitoring of services
Maintains required documentation in adherence to established standards.
