Job summary

Community Care in Wisconsin is seeking a Case Manager to join a mission-driven team dedicated to helping elderly and disabled adults live independently in the community. This full-time, hybrid weekday role in Waukesha County includes an $8,000 sign-on bonus and a comprehensive benefits package.

You will manage an assigned caseload with an RN Case Manager, complete member assessments and care plans, coordinate services, and maintain documentation to meet standards.

Qualifications

• Bachelor’s Degree in a Human Services or closely related area required.
• One year of experience with older adults or persons with physical and/or intellectual disabilities preferred.
• Eligibility for long-term care functional screener certification is required.

This full-time opportunity offers a hybrid weekday schedule and a $8,000 SIGN-ON BONUS. Position located in Waukesha County.

Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
