Job summary

Paris Regional is seeking a Registered Nurse (RN) Case Manager to oversee patient care coordination, discharge planning, and to actively contribute to improving patient journeys with our care team.

Applicants must hold a current state RN license and an associate degree; BLS is required within 30 days, ACLS/PALS within six months, and 1 year of experience is preferred. This position emphasizes quality outcomes and community health.

Qualifications

• Current state RN license and an associate degree from an accredited nursing school.
• Basic Life Support (BLS) certification required within 30 days of hire.
• ACLS and PALS required within six months of hire.
• One year of experience preferred.
• ASLS certification preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness.
• Monitors patient and family satisfaction and addresses questions from patients, families, and payors.
• Participates in discharge planning and coordinates patient transfers and community resources.
• Performs chart reviews to identify issues with service delivery, outcomes, and reimbursement.
