Qualifications

• Must hold an active Registered Nurse (RN) License in the state of employment.
• Minimum two years of recent experience in direct care of elders or adults with disabilities.
• Ability to travel within assigned geographic area; valid driver’s license and automobile insurance is required.
• Experience in case management, care planning and assessment for cases including but not limited to eldercare and disabilities in a health care setting.
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems).
• Additional state-specific qualifications may be required.

• Provide ongoing coaching and support to caregivers via app, including resource referral and reinforcement of best practices.

• Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards.
• Other projects and duties as assigned.

What You Will Do

• Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications
• Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices.
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout
• Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards
• Other projects and duties as assigned.
• Perform other duties as assigned.

What You Will Bring

The pay range for this position is $79,587- $97,464 The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
