Reporting to the Director, Care Management, the Manager, Care Managementis responsible fordirecting care management operations and leading a team to deliver services that engage, motivate, and educate family caregivers to support complex individuals, keeping care at home.This will initially be a workingmanagerrole; the manager will remain hands‑on and, at any given time, may perform similar care management work as members of the team to support operationalneeds, coverage, quality, and service delivery.This individual ensures the delivery of high‑quality services in compliance with all state, program, and accr editation requirements. This individual will manage a team of clinical and non‑clinical care managers, providing case management services across the continuum of care (assessment, care planning, care coordination, and monitoring and evaluating services). In collaboration with key stakeholders, this individual will create action plans in supportofquality improvement efforts and positive health outcomes.

The pay range for this position is $77,000 - $100,000. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
