Careforth, Inc. seeks a Community Health Worker to support elders and disabled individuals in the Worcester area. You will collaborate with a care team to implement person-centered plans and conduct regular home visits, coaching clients to manage chronic conditions and self-care.
You'll build trusted relationships in the community, assist with referrals, travel as needed, and use case management software and mobile apps. Flexibility, bilingual abilities, and a local residency are valued assets.

The Community Health Worker (CHW) works collaboratively with the care team to implement a person-centered plan of care and conduct regularly scheduled homes visits. Responsible for securing, developing and preserving trusting relationships within the community, the CHW will provide consumers and caregivers general support and encouragement. The CHW will achieve this by providing health management plans and goals and coaching consumers to effectively manage their chronic health conditions and self-care.

The pay range for this position is $22.18 - $26.10. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
