Job summary

Hearth Hospice in Savannah is seeking a dedicated Registered Nurse Case Manager (RNCM) for full-time work across Chatham, Effingham, Bryan and Liberty counties. You’ll deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey, with a focus on compassion, clinical excellence, and connection.

Qualifications

• Must hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in computer use and EMR systems.
• CPR certification if required by state.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassion and clinical expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life.
• Deliver nursing care grounded in proven principles and techniques.
• Educate and advocate for patients and families to create personalized care plans.
• Participate in on-call duties as part of the team.

Hearth Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday -Friday, 8 am - 4:30 pm

Territory: Chatham, Effingham, Bryan and Liberty

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
