Job summary

Hearth Hospice is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team serving Hamilton County. Schedule: Mon-Fri 8am-5pm, performing hands-on hospice care, coordinating services, and guiding patients and families through their hospice journey with compassion and clinical excellence.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in med-surg/acute care; hospice exp is a plus.
• Computer proficiency including EMR background.
• CPR certification may be required by state.
• Current driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to referrals with care and expertise, addressing inquiries and requests with compassion.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct care coordination to maintain patient comfort and quality of life.
• Deliver nursing care rooted in proven principles to ease pain and provide solace.
• Educate and advocate for patients and families to create personalized care plans.
• Share call duties as part of a team when needed.

Hearth Hospiceis seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday through Friday 8am to 5pm

Territory: Hamilton County

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation

$65,000 - $77,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
