Zusammenfassung

Mercy is seeking a Registered Nurse for PRN care management across Mercy Hospital Watonga, Kingfisher, and Logan County. You will advocate for patients, identify barriers to care, and develop goals with interventions, coordinating across care settings using telephonic and virtual visits.

You will work with inpatient teams for handoffs, engage patients and families, and apply knowledge of pathophysiology and cardiovascular/respiratory conditions.

Qualifikationen

• Graduation from a registered nursing program with active RN license.
• CCM or RN-BC certification preferred or to be attained within 36 months of hire.
• Experience in care management, patient advocacy and care transitions.

Aufgaben

• Advocates for patients' healthcare needs with emphasis on safety and self-care.
• Provides care management for patients with complex medical conditions and coordinates plans of care.
• Facilitates safe transitions between care settings and collaborates with interdisciplinary teams.
• Engages patients and families to identify goals and support functional independence.
• Documents activities per policy and supports telephonic/virtual visits and home-based care.
