Job summary

Mercy is seeking a registered nurse for PRN case management across Mercy Hospital Watonga and related facilities. The role emphasizes coordinating care, advocating for patients, and facilitating transitions across care settings.

You will work in home or field-based settings and use telehealth tools to engage patients, families, and interdisciplinary teams. CCM or RN-BC certification is preferred and supports career growth.

Qualifications

• Graduation from a registered nursing program.
• Bachelors or higher degree in Nursing.
• CCM or RN-BC certification preferred or attained within 36 months of hire.
• Excellent interpersonal skills and effective communication across diverse teams.

Responsibilities

• Advocates for patients' health care needs with emphasis on safety and self-care.
• Identifies barriers to the patient plan of care and develops goals and interventions.
• Provides care management for patients with complex conditions and coordinates resources.
• Facilitates timely transitions between care levels (home, ED, inpatient, post-acute).
• Works in home/field-based settings and via telephonic or virtual visits with the care team.
