Position Summary

This role involves assessing psychosocial, environmental, and family economic factors affecting patients. The Case Manager acts as a liaison among patients, caregivers, multidisciplinary teams, and third-party payers.

Key Responsibilities
  1. Review and confirm care setting orders using hospital-approved criteria.
  2. Document reviews accurately in the hospital data system.
  3. Coordinate admissions, transfers, and patient flow within the hospital.
  4. Engage with managed care and third-party payers for financial certification.
  5. Manage patient care coordination throughout the healthcare system.
  6. Educate patients and families about rehabilitation benefits and services.
  7. Ensure compliance with laws, regulations, and hospital policies.
Qualifications

Education: Required - RN diploma; Preferred - Associate's degree in nursing.

Experience: Required - 1 year hospital-based patient care; Preferred - Experience in case management or utilization review.

Licenses and Certifications: Required - Active RN license; Preferred - CCM, CPUR, CPUM, or CPHM certifications.

Skills and Abilities
  • Proficient in computer applications and web-based tools.
  • Excellent communication skills.
  • Knowledge of healthcare finance.
  • Flexible schedule and ability to travel as needed.
Physical and Environmental Requirements
