Job summary

Lifepoint Rehabilitation is seeking a dedicated Case Manager/RN at MyMichigan Medical Center Alma. This PRN (as needed) role supports discharge planning, care coordination, and collaboration with the interdisciplinary team to promote optimal patient outcomes.

You will participate in IPoC documentation, schedule family conferences, and coordinate post-discharge services while ensuring CMS guidelines are followed. A current RN license and strong communication skills are essential.

Qualifications

• Must hold an active professional license as RN, Social Worker, Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist.
• Experience with discharge planning or care coordination is preferred.
• Strong communication and teamwork skills are essential.

Responsibilities

• Completes departmental orientation and annual competencies.
• Assist with performance improvement data collection and reporting.
• Collaborate with other departments to coordinate patient care and discharge planning.
• Document IPoC per CMS guidelines and ensure timely workflow entries.
• Schedule family conferences and communicate progress to patients and caregivers.
• Coordinate weekly patient care conferences to refine the treatment plan.
