Job summary

Sunrise Hospital in Las Vegas is seeking a Registered Nurse Case Manager to coordinate the plan of care, manage length of stay, and develop safe discharge plans in collaboration with the multidisciplinary team.

You will promote patient-centered care, conduct admission reviews, and coordinate ongoing care with hospital staff, physicians, and community resources to support optimal outcomes.

Qualifications

• Graduate of an accredited School of Nursing program.
• Current RN license in Nevada.
• Current Basic Cardiac Life Support (BLS) certification.
• Two years in hospital case management with three years nursing experience preferred.
• CMS Rules and Regulations knowledge essential for admission/concurrent reviews.
• Ability to document accurately as directed by the Case Management Plan.

Responsibilities

• Perform admission reviews to determine appropriateness and medical necessity.
• Monitor admissions and ensure timely concurrent reviews.
• Coordinate referrals related to medical necessity, utilization, quality or risk management.
• Plan, coordinate and facilitate discharge needs including Home Health, DME and community resources.
