Job summary

HCA Houston Healthcare Conroe is seeking an RN Case Manager PRN to guide patients and families through the care journey, coordinating resources and coordinating with physicians, nurses, and the care team to ensure safe transitions and high-quality outcomes.

The role emphasizes discharge planning, individualized treatment plans, and active collaboration with multidisciplinary teams to meet each patient’s unique needs while aligning with HCA’s compassionate care standards.

Qualifications

• RN Case Manager experience or clinical nursing experience required.
• Knowledge of care coordination concepts and discharge planning.
• Licensure as a Registered Nurse in the state of practice.

Responsibilities

• Guiding patients and families through program orientation, explaining benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning
• Developing and implementing individualized treatment plans reflecting patient strengths and goals
• Completing psychosocial assessments and discharge planning evaluations with clear documentation
• Collaborating with Rehab Program Director and interdisciplinary team for seamless care
• Promoting HCA Healthcare’s values through every patient and family interaction
