Qualifications

• Active registered nurse licensure in the state of practice.
• Case management experience required.

Responsibilities

• Coordinate end-to-end case management for patients, including care plan development, specialist and diagnostic referral navigation, chronic disease support, patient education, and follow-up to improve health outcomes.
• Collaborate with an integrated primary care team to provide hands-on clinical support (assessments, injections, phlebotomy, IV fluids, vital signs).

Daily Responsibilities For The RN Case Manager

• Coordinate end-to-end case management for patients, including care plan development, specialist and diagnostic referral navigation, chronic disease support, patient education, and follow-up to improve health outcomes and ensure a seamless care experience
• Collaborate closely with an integrated primary care team to provide hands-on clinical support (assessments, injections, phlebotomy, IV fluids, vital signs), while delivering compassionate, patient-centered care through both in-office and virtual interactions
