Job Title: RN Field Case Manager

Are you an experienced Registered Nurse with Case Management/Pediatrics experience looking for a new opportunity with a prestigious healthcare company? Position Summary:

As the RN Field Case Manager, you would be responsible for providing care coordination, medical/disease education and outreach services to participants.

Hours for this Position: Monday-Friday 8:00am-5:00pm

More Insight of Daily Responsibilities:

• Identify care needs and goals based upon assessment findings, analysis of claims data, provider input, participant goals, etc.
• Establish care plan in conjunction with participant, caregiver/parents, and providers
• Convene/facilitate Interdisciplinary Care Team meetings
