Job summary

Bakersfield Family Medical Group, Inc. is seeking a Registered Nurse Case Manager to join the Priority Care Program in Bakersfield, CA.

Under the direction of the Case Management Supervisor, you will assess, plan, implement, and evaluate patient care, coordinating with providers to optimize outcomes and reduce ER utilization. You will conduct annual comprehensive assessments, develop SMART goals, document in NextGen, and educate patients and families about benefits, resources, and care options

Qualifications

• Graduation from an accredited Registered Nursing program.
• Current BLS certification.
• Minimum of two years clinical nursing experience.
• Knowledge of Microsoft Office programs.

Responsibilities

• Assess, diagnose, plan, implement, and evaluate patients in the Priority Care Program.
• Coordinate care with providers and multidisciplinary team.
• Attend patient appointments and carry out provider orders.
• Educate patients and caregivers with resources to prevent ER visits.
• Document in NextGen and maintain accurate brain sheets.
• Monitor acuity levels and perform follow-ups per Level designations.
• Assist with orders including medications, labs, referrals, injections.
• Ensure timely transitions of care back to PCP.
