Job summary

Kern Family Health Care is seeking a qualified Registered Nurse to provide complex case management for high-risk Medicaid members. You will assess needs, develop care plans, and coordinate care across the continuum with providers and KFHC staff.

Responsibilities include education, collaboration with the medical team, and timely documentation. Travel up to 10% is expected; bilingual English/Spanish is preferred.

Qualifications

• Active California RN license is required.
• Associates Degree in Nursing required; Bachelor’s Degree preferred.
• 2 years of full-time clinical experience in relevant settings required.
• Experience in case management or care coordination is a plus.
• Bilingual English/Spanish preferred.

Responsibilities

• Identify high-risk members and assess needs for care planning.
• Coordinate and manage care across outpatient and community services.
• Educate and coordinate outpatient care for selected KFHC members.
• Document care plans and communicate with providers and team.
