Job summary

Ascend Hospice is seeking an RN Case Manager on a per diem basis to support on-call, case management, and field visits. This role involves initial patient assessments, ongoing re-evaluations, and collaboration with the health care team to ensure continuous, coordinated care.

The ideal candidate holds a State RN license with at least two years of nursing experience, hospice experience preferred, and must have a valid driver\'s license with reliable transportation.

Qualifications

• Must have a valid driver\'s license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluate the patient\'s nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the healthcare team, patient and family, to direct care and maintain continuous, well-coordinated service
• Counsel patient and family in meeting end-of-life needs
• Initiate appropriate preventative and palliative nursing procedures
• Communicate with the physician regarding patient needs and changes in condition
• Supervise the Hospice Aides

Hours:

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides
