Ascend Hospice in Massachusetts is seeking an RN Case Manager for a full-time role with a starting salary of $93,600 and a $2,500 sign-on bonus. The position offers a Monday–Friday schedule with no overnights or weekends.

The role includes initial patient/family assessments, regular re-evaluation, care plan development, collaboration with the healthcare team, and supervision of Hospice Aides, with travel required.

Qualifications

• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Completing an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patients nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated.
• Counsels the patient and family in meeting end of life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Supervises the Hospice Aides.
