Job summary

Ascend Hospice & Palliative Care MA in Massachusetts is seeking an RN Case Manager to support on-call, case management, and patient visits. A $5,000 sign-on bonus is offered as part of joining our compassionate care team.

You will assess patients, coordinate care with the healthcare team, and supervise Hospice Aides to ensure continuous, quality end-of-life care for patients and families.

Qualifications

• State RN license required.
• 2 years' nursing experience; hospice preferred.
• Reliable transportation & willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team to provide continuous, well-coordinated care.
• Counsel patient and family in end-of-life needs.
• Initiate preventive and palliative nursing procedures.
• Communicate with the physician about patient needs and condition changes.
• Supervise the Hospice Aides.

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs

• Regularly re-evaluates the patients nursing needs

• Use assessment data to determine care plan needs

• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated

• Counsels the patient and family in meeting end of life needs

• Initiates appropriate preventative and palliative nursing procedures

• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

• Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license

• 2 years' experience as a professional nurse, hospice experience preferred

• Must have a valid Driver's license and reliable transportation

• Must be willing to travel
