Qualifications

• Bachelor's degree in case management or health care.
• Current RN license in state worked.
• BLS certification required.
• 5 years clinical experience.
• Knowledge of care management and utilization review.
• Strong communication and time-management skills.
• Ability to work in interdisciplinary team.

Responsibilities

• Manage patients across health care continuum to optimize outcomes.
• Lead process-improvement activities for patient populations.
• Collaborate on discharge planning and communicate across care continuum.
• Assess medical necessity and patient admissions; resolve delays.
