Qualifications

• RN licensure in Kansas or multi-state licensure
• BLS verification through AHA/Red Cross Healthcare Provider CPR
• Bachelor's degree preferred
• 2 years of case management or 2 years of critical care experience preferred
• Certification in case management or utilization review preferred
• InterQual experience preferred

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients, families and caregivers for treatment planning.
• Develop individualized treatment plans reflecting patient strengths and goals.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every patient interaction.

Education

RN license in KS or multi-state

BLS certification

Bachelor's degree preferred

2 years of case management or 2 years of critical care experience preferred

Case management certification or utilization review preferred
