Formación

• BSN or equivalent experience in acute care.
• Current Missouri RN license and state practice compliance.
• 2+ years in acute care with case management exposure.
• Excellent written and verbal communication skills.
• Strong organizational and team-based collaboration abilities.

• Assess patient needs and determine eligibility for case management services.
• Develop and implement individualized care plans with multidisciplinary team.
