Responsibilities

• Conduct comprehensive patient assessments including medical history, medications, treatment plans and psychosocial needs.
• Collaborate with providers, patients, and families to develop and implement individualized care plans.
• Monitor progress, adjust care plans as needed, and communicate changes to the team.
• Serve as main contact between patient and providers to ensure seamless communication and care coordination.
• Maintain up-to-date records and case notes in compliance with confidentiality standards.
