Job summary

Grand Strand Medical Center is seeking a dedicated Case Manager to support patients and families through every step of care. You will coordinate services, develop individualized care plans, and partner with physicians, nurses, and leaders to ensure safe transitions and consistent quality across the continuum of care.

You will guide education for patients and families, document assessments, and collaborate with the Rehab Program Director and interdisciplinary team to ensure patient-centered care.

Qualifications

• RN required
• Basic Cardiac Life Support within 30 days of start date
• No travel required
• 1 year of experience required

Responsibilities

• Support patients and families through care journeys; coordinate services and develop care plans reflecting patient needs
• Collaborate with physicians, nurses and leaders to ensure safe transitions and clear communication
• Document psychosocial assessments and discharge planning evaluations for the care team
• Provide education for patients, families and caregivers to participate in treatment planning
• Assist in coordinating program orientation and discharge criteria and patient rights
