Qualifications

• Associate Degree in Nursing is required.
• BSN is preferred.
• Current and valid Registered Nurse license in state of practice or compact.
• BLS certification is required within facility policy time frame.
• ACM Certification as required by facility policy.
• Minimum 1–2 years of clinical experience in acute hospital/clinic settings.
• Previous case management experience is preferred.

Responsibilities

• Assess, plan, coordinate, and monitor healthcare services to meet patient needs.
• Perform discharge planning and arrange post-discharge services as needed.
• Collaborate with interdisciplinary team to ensure quality care and safe transitions.
• Communicate with insurers for authorization and approvals.
• Document activities and education in the EMR for administrative reporting.
• Participate in utilization review and performance improvement initiatives.
• Conduct follow-up calls post-discharge as applicable.
• Advocate for patients and coordinate with providers and families.
