Shenandoah Community Health (SCH) is seeking an RN Case Manager in Martinsburg, WV to join our Care Coordination team. This community-based nursing role helps patients overcome barriers to care, connect with needed resources, and improve health outcomes.

Five 8-hour days with no nights or weekends. You will provide outreach, education, and care coordination across clinic, home, and community settings, assessing barriers such as transportation, housing, and health literacy, while connecting

Qualifications

• Current nursing license in the state where services are provided; RN preferred.
• Experience in healthcare, care coordination, case management, or social services preferred.
• BLS/CPR certification required or ability to obtain within required timeframe.
• Strong communication, organization, documentation, and teamwork skills.
• Valid driver’s license and ability to travel locally as needed.
• Bilingual English/Spanish skills preferred but not required.

Responsibilities

• Provide patient outreach, education, and care coordination in clinic, community, and patient home settings.
• Assess barriers to care such as transportation, food access, housing, insurance, medications, and health literacy.
• Connect patients with medical, behavioral health, social service, and community resources.
• Collaborate with providers, care teams, social workers, and community partners.
