Job summary

Singing River Pascagoula Hospital is seeking a dedicated Case Manager to improve clinical quality and resource management. You will coordinate patient care across ages within the health system, participate in discharge planning, and ensure clear communication between inpatient and outpatient teams.

The role requires a graduate of an accredited nursing program, an active RN license in Mississippi, BCLS and TB certifications, plus 2 years of nursing experience related to Case Management.

Qualifications

• Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Two years of nursing experience in ambulatory or acute care relevant to Case Management.
• Current RN license to practice in Mississippi; BCLS certification required; TB certification as applicable.
• De-escalation training completed by 90 days; strong communication and organizational skills.

Responsibilities

• Coordinate patient care to a diverse population within Singing River Health System.
• Participate in discharge planning meetings to address barriers and ensure smooth transitions.
• Maintain collaboration between inpatient care management and outpatient case management teams.
