Job summary

AccentCare, Inc. is seeking an RN Case Manager to oversee patient care plans in Petaluma, CA. This full-time role runs Mon-Fri, 8 am–5 pm, with on-call duties as needed. You’ll manage care from admission through discharge and coordinate the care team to ensure quality patient outcomes.

Qualifications

• RN license in the state of operation is required.
• Minimum of 1 year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial admission assessment and collect patient history.
• Identify nursing diagnoses and coordinate care with the team.
• Review medications for safety, allergies and interactions.
• Document assessments, progress and discharge notes timely.
• Inform physicians of changes and recommended interventions.
